    Water Contaminants Detection Using Sensor Placement Approach in Smart Water Networks

    Incidents of water pollution or contamination have occurred repeatedly in recent years, causing significant disasters and negative health impacts. Water quality sensors need to be installed in the water distribution system (WDS) to allow real-time water contamination detection to reduce the risk of water contamination. Deploying sensors in WDS is essential to monitor and detect any pollution incident at the appropriate time. However, it is impossible to place sensors on all nodes of the network due to the relatively large structure of WDS and the high cost of water quality sensors. For that, it is necessary to reduce the cost of deployment and guarantee the reliability of the sensing, such as detection time and coverage of the whole water network. In this paper, a dynamic approach of sensor placement that uses an Evolutionary Algorithm (EA) is proposed and implemented. The proposed method generates a multiple set of water contamination scenarios in several locations selected randomly in the WDS. Each contamination scenario spreads in the water networks for several hours, and then the proposed approach simulates the various effect of each contamination scenario on the water networks. On the other hand, the multiple objectives of the sensor placement optimization problem, which aim to find the optimal locations of the deployed sensors, have been formulated. The sensor placement optimization solver, which uses the EA, is operated to find the optimal sensor placements. The effectiveness of the proposed method has been evaluated using two different case studies on the example of water networks: Battle of the Water Sensor Network (BWSN) and another real case study from Madrid (Spain). The results have shown the capability of the proposed method to adapt the location of the sensors based on the numbers and the locations of contaminant sources. Moreover, the results also have demonstrated the ability of the proposed approach for maximising the coverage of deployed sensors and reducing the time to detect all the water contaminants using a few numbers of water quality sensor

    Awareness of school students on sexually transmitted infections (STIs) and their sexual behavior: a cross-sectional study conducted in Pulau Pinang, Malaysia

    <p>Abstract</p> <p>Background</p> <p>Sexually transmitted Infections (STIs) rank among the most important health issues for the people especially the young adults worldwide. Young people tend to engage in sexual activity at younger ages in the past decade than in the 1970s, and 1980s. Knowledge is an essential precursor of sexual risk reduction. A cross-sectional study was conducted in Pulau Pinang, Malaysia, to produce the baseline information about school students' awareness and perception about sexually transmitted Infections (STIs) and their sexual activity to help establish control and education programmes.</p> <p>Methods</p> <p>Students from form 4 (aged between 15 to 16 years), form 5 (aged between 16 to 17 years) and form 6 (aged between 18 to 20 years) in their class rooms were approached and asked to complete self administered and anonymous pre-validated questionnaires. SPSS for windows version 13 was used to analyze the results statistically and results were presented in tabular form.</p> <p>Results</p> <p>Data was collected from 1139 students aged between 15 to 20 years, 10.6% of which claimed that they never heard about STIs. Sexual experience related significantly with gender, race, and education level. Approximately 12.6% claimed to have sexual experience of which 75.7% had their sexual debut at 15-19 years and 38.2% were having more than 3 partners. Sexual experience was found to be significantly associated with gender (<it>p </it>= 0.003), ethnicity (<it>p </it>= 0.001) and education level (<it>p </it>= 0.030). However, multiple partner behaviour was significantly associated only with gender (<it>p </it>= 0.010). Mean knowledge score was 11.60 ± 8.781 and knowledge level was significantly associated with religion (p = 0.005) education level (<it>p </it>= 0.000), course stream (<it>p </it>= 0.000), socioeconomic class (<it>p </it>= 0.000) and sexual experience (<it>p </it>= 0.022).</p> <p>Conclusions</p> <p>It was concluded that school students have moderate level of knowledge about STIs although they are sexually active. Interventions such as reinforcing the link between STIs and HIV/AIDS, assessing the current status of sexuality education in schools and arranging public talks and seminars focusing on STIs prevention education are needed to improve their awareness.</p

    An overview of the utilisation of microalgae biomass derived from nutrient recycling of wet market wastewater and slaughterhouse wastewater

    Microalgae have high nutritional values for aquatic organisms compared to fish meal, because microalgae cells are rich in proteins, lipids, and carbohydrates. However, the high cost for the commercial production of microalgae biomass using fresh water or artificial media limits its use as fish feed. Few studies have investigated the potential of wet market wastewater and slaughterhouse wastewater for the production of microalgae biomass. Hence, this study aims to highlight the potential of these types of wastewater as an alternative superior medium for microalgae biomass as they contain high levels of nutrients required for microalgae growth. This paper focuses on the benefits of microalgae biomass produced during the phycore-mediation of wet market wastewater and slaughterhouse wastewater as fish feed. The extraction techniques for lipids and proteins as well as the studies conducted on the use of microalgae biomass as fish feed were reviewed. The results showed that microalgae biomass can be used as fish feed due to feed utilisation efficiency, physiological activity, increased resistance for several diseases, improved stress response, and improved protein retention

    Pooled analysis of WHO Surgical Safety Checklist use and mortality after emergency laparotomy

    Background The World Health Organization (WHO) Surgical Safety Checklist has fostered safe practice for 10 years, yet its place in emergency surgery has not been assessed on a global scale. The aim of this study was to evaluate reported checklist use in emergency settings and examine the relationship with perioperative mortality in patients who had emergency laparotomy. Methods In two multinational cohort studies, adults undergoing emergency laparotomy were compared with those having elective gastrointestinal surgery. Relationships between reported checklist use and mortality were determined using multivariable logistic regression and bootstrapped simulation. Results Of 12 296 patients included from 76 countries, 4843 underwent emergency laparotomy. After adjusting for patient and disease factors, checklist use before emergency laparotomy was more common in countries with a high Human Development Index (HDI) (2455 of 2741, 89.6 per cent) compared with that in countries with a middle (753 of 1242, 60.6 per cent; odds ratio (OR) 0.17, 95 per cent c.i. 0.14 to 0.21, P <0001) or low (363 of 860, 422 per cent; OR 008, 007 to 010, P <0.001) HDI. Checklist use was less common in elective surgery than for emergency laparotomy in high-HDI countries (risk difference -94 (95 per cent c.i. -11.9 to -6.9) per cent; P <0001), but the relationship was reversed in low-HDI countries (+121 (+7.0 to +173) per cent; P <0001). In multivariable models, checklist use was associated with a lower 30-day perioperative mortality (OR 0.60, 0.50 to 073; P <0.001). The greatest absolute benefit was seen for emergency surgery in low- and middle-HDI countries. Conclusion Checklist use in emergency laparotomy was associated with a significantly lower perioperative mortality rate. Checklist use in low-HDI countries was half that in high-HDI countries.Peer reviewe

    Global variation in anastomosis and end colostomy formation following left-sided colorectal resection

    Background End colostomy rates following colorectal resection vary across institutions in high-income settings, being influenced by patient, disease, surgeon and system factors. This study aimed to assess global variation in end colostomy rates after left-sided colorectal resection. Methods This study comprised an analysis of GlobalSurg-1 and -2 international, prospective, observational cohort studies (2014, 2016), including consecutive adult patients undergoing elective or emergency left-sided colorectal resection within discrete 2-week windows. Countries were grouped into high-, middle- and low-income tertiles according to the United Nations Human Development Index (HDI). Factors associated with colostomy formation versus primary anastomosis were explored using a multilevel, multivariable logistic regression model. Results In total, 1635 patients from 242 hospitals in 57 countries undergoing left-sided colorectal resection were included: 113 (6·9 per cent) from low-HDI, 254 (15·5 per cent) from middle-HDI and 1268 (77·6 per cent) from high-HDI countries. There was a higher proportion of patients with perforated disease (57·5, 40·9 and 35·4 per cent; P < 0·001) and subsequent use of end colostomy (52·2, 24·8 and 18·9 per cent; P < 0·001) in low- compared with middle- and high-HDI settings. The association with colostomy use in low-HDI settings persisted (odds ratio (OR) 3·20, 95 per cent c.i. 1·35 to 7·57; P = 0·008) after risk adjustment for malignant disease (OR 2·34, 1·65 to 3·32; P < 0·001), emergency surgery (OR 4·08, 2·73 to 6·10; P < 0·001), time to operation at least 48 h (OR 1·99, 1·28 to 3·09; P = 0·002) and disease perforation (OR 4·00, 2·81 to 5·69; P < 0·001). Conclusion Global differences existed in the proportion of patients receiving end stomas after left-sided colorectal resection based on income, which went beyond case mix alone

    Do silvopastoral management practices affect biological pest control in oil palm plantations?

    The reliance of conventional oil palm production on agrochemicals to control understory vegetation has caused devastating impacts on the environment. Overuse of chemical herbicides leads to the decline of native biodiversity and production-related ecosystem services, including biological pest control. In contrast to herbicide applications, livestock integration is a chemical-free approach to eradicate weeds without negative consequences on the environment. In this sentinel prey experiment, we assessed the predation pressure (based upon the number of bite marks left by natural predators such as arthropods, birds, and mammals) in three oil palm plantations with different weeding practices (i.e., rotational grazing, free-range grazing, and conventional weeding) located on the south-west part of Peninsular Malaysia. We also investigated the effects of site level characteristics on the predation pressures of artificial caterpillars. Across all types of weeding systems, we found that the majority of the predation attempts on the deployed artificial caterpillars were made by arthropods (532 attempts), whereas mammals (66 attempts) and birds (60 attempts) played much smaller roles. Our results showed that predation pressures were consistent across sites indicating that oil palm-cattle integrated plantations (either free-range or rotational grazing systems) would experience similar level of pest control services as conventional herbicide-sprayed plantations. Our results also indicate the influence of site-level habitat variables (i.e., mean of palm heights, canopy cover, and elevation) on predation pressures. Our results suggest that livestock-oil palm integration may sustain natural predation with little or no herbicide input

    Assessing the validity of a self-administered food-frequency questionnaire (FFQ) in the adult population of Newfoundland and Labrador, Canada

    Background: The Food- Frequency Questionnaire (FFQ) is a dietary assessment tool frequently used in large-scale nutritional epidemiology studies. The goal of the present study is to validate a self-administered version of the Hawaii FFQ modified for use in the general adult population of Newfoundland and Labrador (NL). Methods: Over a one year period, 195 randomly selected adults completed four 24-hour dietary recalls (24-HDRs) by telephone and one subsequent self-administered FFQ. Estimates of energy and nutrients derived from the 24-HDRs and FFQs were compared (protein, carbohydrate, fibre, fat, vitamin A, carotene, vitamin D, and calcium). Data were analyzed using the Pearson’s correlation coefficients, cross-classification method, and Bland–Altman plots. Results: The mean nutrient intake values of the 24-HDRs were lower than those of the FFQs, except for protein in men. Sex and energy-adjusted de-attenuated Pearson correlation coefficients for each nutrient varied from 0.13 to 0.61. Except for protein in men, all correlations were statistically significant with p < 0.05. Cross-classification analysis revealed that on average, 74% women and 78% men were classified in the same or adjacent quartile of nutrient intake when comparing data from the FFQ and 24-HDRs. Bland–Altman plots showed no serious systematic bias between the administration of the two instruments over the range of mean intakes. Conclusion: This 169-item FFQ developed specifically for the adult NL population had moderate relative validity and therefore can be used in studies to assess food consumption in the general adult population of NL. This tool can be used to classify individual energy and nutrient intakes into quartiles, which is useful in examining relationships between diet and chronic disease
